Your monthly membership includes a wide range of medical services, without any copays or additional fees. We also work incredibly hard to obtain affordable pricing for services that need to be handled outside our office. We share these wholesale prices with our patients, so that they can stop worrying about medical bills and focus on keeping themselves and their families healthy.

Payment Options
*We DO NOT bill any insurance in our practice! We are a membership medical practice!
- Patients WITH insurance are welcome to join our practice and use it for their needs outside of the Primary Care office!
- Patients WITHOUT insurance are welcome to join our practice as well!
*Enrollment Fee: One-time non-refundable enrollment fee of $100 per person or $200 per family.
Includes: obtaining prior medical records, comprehensive initial physical exam, and onboarding process
*WAIVED ENROLLMENT FEE with:
- Annual Membership (instead of monthly)
- Families of 4+ members (Parents and their children only)
- College students (ages 19-26)
- Small Businesses (with 5+ employees enrolled)
*Children’s Membership price:
Requires an adult membership (Full price without an adult membership)
Membership Fees
Membership fee(s) is started upon enrollment. Membership fees are paid by the calendar month on an ongoing basis until cancellation. Payment of membership fees is required to remain an active member and receive service.
All members must enroll for Auto-Pay of invoices with a Credit or pin-less Debit Card. Auto-pay date may be selected for any day of the month during set-up. Memberships require NO LONG-TERM CONTRACTS, BUT a minimum of 3 months commitment is required for the monthly plan. However, upon membership cancellation, re-enrollment is required.
Cancellation
Monthly memberships may be canceled, for any reason, with 30 days notice prior to the next month’s automatic payment. All charges, including membership fees, will continue to accrue until notice of cancellation is given.
Re-Enrollment
If a member discontinues membership – by choice or lack of payment – the member may contact LIVING AT YOUR FINEST WELLNESS to discuss if re-enrollment is possible. A $250 fee will be issued upon member re-enrollment.
Service Fees
All services not covered by membership fees (e.g. home visit, non-covered labs, medications dispensed in our office etc.) will be added to the next month’s autopay bill.
